Overview
The table shows the data fields required for each PEAK continuing knowledge declaration submitted by practising licence holders (and the corresponding data entry format): activity name (enter a brief title), activity reporting cycle (select an option from a drop-down menu), activity start date (select an option from a date picker), activity end date (select an option from a date picker), activity type (select an option from a drop-down menu), activity objective (select an option from a drop-down menu) and activity duration (enter an answer in 15-minute increments).
